On February 22, 2024, Licensing Program Analysts (LPAs), Paul Garcia and Monica Lopez conducted an unannounced Annual Required Inspection and was met by Licensee, Maria Rodriguez. Also present was her assistant. Licensee is Spanish Speaking and Monica Lopez assisted with interpretation. Days and hours of operation are Monday - Friday 23 1/2 hours. No weekend care provided. The home has a working telephone service and LPA confirmed the phone number is (661) 396-1720.

LPA toured the home inside and outside and a census was taken. A facility sketch was reviewed, and it was observed to not be current. Licensee confirmed that the children’s playroom (converted garage), bathroom, sleeping room, and the outdoor backyard play yard are used for providing care and are accessible to children. All other rooms are off-limits and made inaccessible by use of spinning doorknobs. The outdoor play area in the backyard is fenced and there are no hazards to children present. There is no swimming pool or other bodies of water on the premises. During the inspection LPAs observed a bottle of bleach and a spray bottle of Lysol that contained liquid confirmed by smell, located in the children’s designated bathroom that was accessible to all children in care. LPAs also observed a large can of Lysol disinfectant aerosol can in the children’s sleeping room that was accessible to children.

This is a single level home. There are no firearms or ammunition on the premises and there are no fireplaces or open face heaters in the home. There is a working fire extinguisher, smoke detector, carbon monoxide detector and adequate heating and ventilation for safety and comfort. Safe toys and play equipment were observed. During this inspection LPAs observed a baby walker in the accessible sleeping area of the childcare home. While inspecting the sleeping play pens, LPAs observed additional unserviceable sleep mats. Upon further inspection, LPAs observed a white dry substance that appeared to be milk under the mattress indicating the area has not been cleaned for some time. LPAs also found a small toy finger approximately one inch in length posing a choking hazard to children while sleeping.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children. Capacity as specified on the license is being maintained.

LPAs reviewed a sample of children’s files and observed files were incomplete. Two (2) of five (5) children’s files were missing LIC9227 and Licensee failed to document infant 15 minute checks as required per the conditions of the license. LPAs determined 15 minute checks have not been conducted in over a year. Licensee’s Mandated Reporter Training was completed on January 6, 2024. Licensee’s pediatric CPR/First Aid expires on April 21, 2025. A review of staff records indicates that Two (2) of four (4) staff did not have form LIC9108 and LIC9052. However, all employees and/or volunteers had immunization records on file for influenza, pertussis and measles.

All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) are not currently being provided. Incidental Medical Services (IMS) policy was discussed. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee confirmed that there are no Registered Sex Offenders living in the facility.

LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiencies are being cited: (see next page, 809 D)

LPA Paul Garcia informed licensee Maria Rodriguez that this report dated February 22, 2024 documents two (2) Type A citations which shall be posted for 30 consecutive days as there is immediate risks to the health, safety, or personal rights of children in care.

Also, LPA Paul Garcia informed Maria Rodriguez she is required to provide a copy of this licensing report dated February 22, 2024, that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted and this report was reviewed with the facility representative Maria Rodriguez

A notice of site visit was given and must remain posted for 30 days.

Appeal Rights were discussed and issued.
