25 | On 01/15/2025 Licensing Program Analyst (LPA) Lady Cabrera arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Director Jacinta Zepeda. LPA toured the facility inside and out and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 10/22/2024 regarding a child receiving an injury on the outdoor play area.
During today’s inspection, LPA reviewed the play yard and the location of staff at the time of the incident. Play structure is surrounded by playground rubber mulch. There is adequate cushioning in the play yard to absorb falls in and around the high climbing areas. Staff reported children are only allowed to run in rubber mulch area. Staff 1 was supervising and monitoring Zone 1/Zone 2 and Staff 2 was supervising Zone 3 at the time of the incident. There were 15 children present. Child 1 was being chased by Child 2 in mulch area, when Child 2 looked back, tripped, and hit the right side of the child's nose on the slide. Staff 1 witnessed the incident and observed Child 1 having a nosebleed. Child 1 cried and informed Staff 1 "it hurt." Staff 1 immediately requested help from Staff 3 and Child 1 was assisted to the office. Parents were notified immediately. Parents picked up Child 1 shortly and transported child to be medically evaluated. Parent provided medical note to facility indicating Child 1 was to return with no restrictions.
Based on the interviews conducted and LPA observation it was determined that Child 1 sustained an injury after tripping and hitting the edge of the slide. Staff did a sweep of the yard before the children went outside and there was no tripping or slipping hazards present at the time of the incident. Staff provided first aid and care to the injured child.
This appears to be an isolated incident and staff took appropriate measures to address the child and followed the appropriate policies, regulations, and reporting requirements.
