25 | On 3/14/22 a case management inspection was conducted today by Licensing Program Analyst (LPA), Caroline Harris. LPA met with Director, Janae Hansen-Grounds to discuss the incident which occurred on 3/4/22. LPA interviewed Mrs. Hansen-Grounds and teacher, Lori Maguregui and a census was taken.
On 3/4/22, at approximately 9:30 a.m., a child was upset about his/her mask being wet and was laying on the floor crying. Teacher, Lori Maguregui calmed the child down and reached out both hands to help the child stand up. The child took Ms. Maguregui's hands and stood up but then stated that she hurt him/her. Ms. Maguregui and Mrs. Hansen-Grounds both checked the child but he/she did not want to be touched. The child was pointing to his/her forearm. The child’s father was notified and he/she was picked up at approximately 9:50 a.m., and was taken to Urgent Care. It was later reported that the child was diagnosed with Nursemaid's Elbow. The licensee reported the incident to Community Care Licensing.
Based on the information obtained, LPA determined Licensee handled the incident correctly and reporting requirements were met. After interviewing staff and reviewing facility records, LPA determined Licensee took appropriate measures to address the child's injury, following proper policies and procedures and no regulations were violated. Per California Code of Regulations Title 22, Division 12, Chapter 1, no deficiency was cited during today's visit. An exit interview was conducted with Director, Janae Hansen-Grounds.
A copy of this report, along with the LIC 9213, Notice Of Site Visit Form was provided to Mrs. Hansen-Grounds. The LIC 9213 Notice Of Site Visit Form is required to be posted for 30 days. |
