25 | On 05/26/2022, Licensing Program Analyst (LPA) Robert Gutierrez met with Director Janae Hansen-Grounds for an unannounced case management incident inspection. LPA toured the facility and a census was taken. The purpose of today's inspection was to address an unusual incident, reported to the Fresno Community Care Licensing (CCL) office. This incident occurred at the facility in playground on 05/09/2022.
This incident involved a day-care child who was running down hill in the playground, fell and landed on his/her shoulder. While playing ball with other children in care, Child #1 (C1) and several other children started to chase after a ball that was going downhill. While running downhill, C1 fell and landed on his/her right shoulder. C1 then approached Staff #1 (S1) and informed them that his/her right shoulder hurt. C1 was accessed for injuries by S1. S1 did not observe any redness, bruising or swelling but still applied an ice pack to the injured area. C1 was cleared and returned to playing outside. Per staff, C1 did not complain about the injury while in care. In addition, staff checked on C1 throughout the day to ensure he/she was doing okay. Later that evening, C1 complained that his/her shoulder hurt. The Legal Guardian (LG) of C1 checked his/her right shoulder and noticed bruising. The following day (05/10/2022) C1 was taken to a local urgent care and diagnosed with an injury on his clavicle bone.
During the time of the incident, 56 children were in the playground and a total of 5 teachers were present and supervising children. Based on interviews conducted, these staff members were in their correct zones to monitor and supervise children.
Based on the information obtained, LPA determined Licensee handled the incident correctly and reporting requirements were met. After interviewing staff and reviewing facility records, LPA determined Licensee took appropriate measures to address the child's injury, following proper policies and procedures and no regulations were violated.
Per California Code of Regulations Title 22, Division 12, Chapter 1, no deficiency was cited during today's visit. An exit interview was conducted with Director, Janae Hansen-Grounds. |
