25 | On this date, Licensing Program Analysts (LPAs) Gloria Reyes and Roman Iglesias met with Director, Sarah Vinson for an unannounced Case Management- Incident inspection. LPAs toured the facility. There are no children present. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office regarding an incident that occurred on 03/02/22, around 2:00 PM, on the Canyon Hills Youth Bus, Child #1 allegedly took Child #2’s hand by the wrist and forced Child #1, to touch Child #1’s private area. LPAs toured the facility and a census was taken.
An internal investigation was conducted by the facilities Manager, Christine Richardson. Interviews were conducted with parents of Child #1 and Child #2 and all children on the youth bus. Investigation revealed no claims to support what each child reported.
LPAs collected a copy of the internal investigation conducted by the facility. Director Vinson stated to prevent this type of incident from occurring in the future she will ensure a seat assignment on the bus.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited.
An exit interview conducted with Director, Sarah Vinson. A copy of this report and appeal rights are provided and discussed. A Notice of Site Visit Form is posted to parent's board and must remain posted for 30 days.
