25 | On November 14, 2024, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced Case Management Incident inspection. LPA met with Administrator, Brianna Contreras and toured the facility and took a census. LPA informed Contreras that the purpose for today’s inspection was to follow-up on an incident that was self-reported via an unusual incident report to Community Care Licensing (CCL) on November 8, 2024, where child 1 was left outside unsupervised for less than one minute during a transition from the outdoor play yard to the classroom. S1 from an adjacent toddler classroom observed the child, who was outside on the playground, and brought her indoors. S2 was outside with 12 children at the time of the incident.
As a result of this incident, Brianna Contreras held a 1 to 1 training with all staff on November 9, 2024, pertaining specifically to improving strategies on supervision.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D)
Upon receipt of a type A violation, the licensee shall post and provide copies of this licensing report to parents/guardians of the children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224 was given to the licensee.
An exit interview was conducted with Brianna Contreras and a copy of this report was provided.
This report shall be made available to the public upon request.
LIC 9213 Notice of Site Visit was issued and is required to be posted for 30 days.
