32 | There were water leaks at the base of the two toilets. LPAs also observed soiled sheets on the floor near the children’s sink of the restroom area.
LPAs also observed in Room #3, soiled stains on the carpet, missing base panels throughout the classroom, sticky tile floors throughout the classroom, food on the floor, dirty children chairs, missing panel edge to a child’s table, a dirty workstation counter, chipped paint by workstation, a missing cover near exit door by window, a broken knob at the kitchen sink area, and an accessible exposed vent of a water dispenser.
LPAs observed in Room #4, sticky tile floors, a stained carpet, a broken table leg stand and two broken panels that were 2 to 3 inches in length.
LPAs observed in Room #5, there was feces on the toilet seat and on the floor, bathroom cabinet at the base was corroding, and a dirty oven toy. Staff immediately removed and clean the feces.
Based upon observations and information gathered through interviews, the preponderance of evidence standard has been met, and the above allegation is found to be SUBSTANTIATED.
Exit interview conducted and report was reviewed with Assistant Director, Candice Lujan. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
