On 11/17/2023, Licensing Program Analysts (LPAs) Gloria Reyes and Lady Cabrera arrived at the facility to conduct an unannounced complaint inspection to gather information to investigate the above allegation. A complete file review was conducted prior to today's visit. LPAs met with Assistant Director, Candice Lujan. LPAs toured the facility both inside and outside. There are three preschool classrooms (Room #3, Room #4, and Room #5). LPAs explained the allegation and a census was taken. LPAs interviewed Assistant Director and some staff. LPAs were provided with some documents.

Investigation showed for the above allegation that upon entering Room #3, LPAs could smell a strong unpleasant musty odor throughout the classroom. There was mold residual on a large folding play mat. Assistant Director removed mat immediately. As LPAs entered the restroom in Room #3, LPAs could also smell soiled diapers coming from the trash bin. LPAs observed that the lid cover to the trash bin was broken. LPAs also observed missing toilet base stand covers on two toilets.

There were water leaks at the base of the two toilets. LPAs also observed soiled sheets on the floor near the children’s sink of the restroom area.

LPAs also observed in Room #3, soiled stains on the carpet, missing base panels throughout the classroom, sticky tile floors throughout the classroom, food on the floor, dirty children chairs, missing panel edge to a child’s table, a dirty workstation counter, chipped paint by workstation, a missing cover near exit door by window, a broken knob at the kitchen sink area, and an accessible exposed vent of a water dispenser.

LPAs observed in Room #4, sticky tile floors, a stained carpet, a broken table leg stand and two broken panels that were 2 to 3 inches in length.

LPAs observed in Room #5, there was feces on the toilet seat and on the floor, bathroom cabinet at the base was corroding, and a dirty oven toy. Staff immediately removed and clean the feces.

Based upon observations and information gathered through interviews, the preponderance of evidence standard has been met, and the above allegation is found to be SUBSTANTIATED.

Exit interview conducted and report was reviewed with Assistant Director, Candice Lujan. Appeal rights were provided.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
