25 | On 6/8/2023, Licensing Program Analyst (LPA) Jessika Thompson met with Director Laurie Curbow for an unannounced Case Management Inspection. The purpose of this inspection was to address an unusual incident reported to Fresno Community Care Licensing office on 6/5/2023 regarding a child that was left unattended at the facility on 6/5/2023. Today, LPA reviewed the incident report with Director Laurie Curbow and Assistant Director Candace Lujan. Additionally, LPA spoke with attending staff members.
It was reported that while two teachers and one teacher's aide were transitioning 15 children from Classroom #5 to the facility's gymnasium, Child #1 was mistakenly left in Classroom #5, without supervision, for approximately 15 minutes. Child #1 was discovered by a front desk support staff member who entered Classroom #5 and observed Child #1 to be lying down alone in the classroom, asleep. Facility staff were being overseen by Assistant Director Candace Lujan on this date as Director Laurie Curbow was not present.
LPA Thompson informed Director Curbow that this report dated 6/8/2023 documents one Type A citation, which shall be posted for 30 consecutive days as there was an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Thompson informed Director Curbow to provide a copy of this licensing report dated 6/8/2023 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Reports (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited: (see next page, LIC 809 D). Director Curbow was provided a copy of appeal rights. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Curbow.
