On 12/12/2024, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to deliver investigation finding to the allegation. LPA met with Licensee and took a census.
The investigation consisted of a review of records, copy of children’s roster, interviews with Director, staff and additional pertinent information obtained during the investigation. Regarding the allegation, it was determined that 2 infants walked out of the multipurpose room through the backdoor. Interviews and a review of documentation determined that the infants walked out and/or left the multipurpose room. It is noted that the alarms on the exit doors to the multipurpose room were disabled and that door to which the infants exited could not be pushed by the infants themselves, thus, appearing that the door was propped open by an adult for the infants to exit to the hallway. Per staff interviews, the infants may have been left unattended for approximately 5-10 minutes. A staff member who no longer works at the facility “reportedly” found the 2 infants in the hallway and alerted the Director.

The circumstances that led to infants exiting the multipurpose room and the inability to account for the whereabouts of the infants for approximately 5-10 minutes posed an immediate threat to the health and safety of the 2 infants.

Based on the information obtained, there is a preponderance of the evidence to prove Staff did not provide adequate supervision to daycare children.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, the following deficiency Type A is being
cited (see 9099-D)

Exit interview conducted and report was reviewed with Director, Laurie Curbow. Appeal rights were provided.

LPA Denisia Jimenez informed licensee, Laurie Curbow that this report dated 12/12/2024 documents 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Denisia Jimenez informed the licensee to provide a copy of this licensing report dated 12/12/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224

Acknowledgement of Receipt of Licensing Reports was given to Licensee.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Responsibility for Providing Care and Supervision: The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met as evidenced by: 2 infants left the multipurpose room for approximately 5-10 minutes without staff knowing the infants had left and did not know how the infants got out.
