13 | On 03/08/2023, An unannounced complaint inspection was conducted today by Licensing Program Analyst (LPA), Jose Penate. LPA met with Licensee, Hailey Simon. During investigation, LPA interviewed staff, and gathered information.
With the allegation of, Staff did not prevent day care child from engaging in inappropriate behaviors, was investigated and based off interviews conducted and information received, there is evidence and it indicates when a child is having difficulty at the facility and being address with the parent after mulitple occasions. The facility is not following the School Operating Policies located on subdivision E of the agreement, which in term violates the health and safety of all children in care.
This agency has investigated the complaint. After information gathered the above allegation has determined that licensee is not following policy in place to ensure the safety and personal rights of all children in care are being followed. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12, deficiency cited (see LIC 9099-D).
An exit interview was conducted with Licensee, Hailey Simon.
A printed copy of this report as well as appeal rights were provided Licensee, Hailey Simon at the conclusion of the visit.
A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days.
