25 | On March 27, 2024, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced Case Management incident inspection. LPA met with Director Rosalva Banuelos and toured the facility and took a census. LPA informed Director Rosalva that the purpose for today’s inspection was to follow-up on an incident that was self-reported via an unusual incident report to Community Care Licensing (CCL) on March 25, 2024, where a child was found to be unsupervised in classroom twenty-six (26) for approximately one minute on March 19, 2024.
On March 19, 2024, at approximately 11:00 AM there were a total of fourteen (14) children, and three (3) staff present in classroom twenty-six (26). While the teachers were transiting the children from the classroom to outdoor play, S1 conducted a physical count of the children present outdoors and discovered that one child was missing.
S1 asked S2 to check the recently departed classroom for the missing child. When S2 opened the door to room twenty-six, (26) she observed C1 standing near a cabinet unsupervised.
Director Rosalva held a meeting with her staff on March 21, 2024 to focus on improving strategies pertaining to supervision.
