25 | On November 13, 2024, Licensing Program Analyst (LPA) Paul Garcia conducted an unannounced Case Management Incident inspection. LPA met with Director Rosalva Banuelos. LPA Garcia toured the facility and took a census. The purpose of today’s inspection was to discuss an unusual incident that was reported to Community Care Licensing (CCL) on October 25, 2024, where Child 1 (C1) stated a teacher “hurt her” during an incident in classroom #4.
At approximately 9:15 AM (C1) was having a moment and started throwing toys and was not following directions. S2 escorted all other children out of the classroom to a play yard to assist in de-escalating the situation. Staff then observed C1 pulling his/her own hair and scratching his/her neck on both sides that left visible marks. S1 then requested assistance via radio. S3 responded and assisted in the de-escalation of C1.
Director was interviewed and S3 was interviewed and provided additional details of what she observed on October 25, 2024.C1 was also interviewed with Rosalva present.
Due to this incident, Director Rosalva has implemented an action plan with child’s authorized representative and other supporting agencies to assist C1 in regulating his/her emotions. The action plan encompasses using simple language, the use of visual aids, modeling positive behavior, teaching social-emotions skills, creating a structured environment and the use of redirection and gentle consequences.
LPA determined all staff at this facility took appropriate actions. This facility will continue to provide additional training to all their childcare staff to reenforce staffs knowledge in working with children in a crisis situation.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
A notice of site visit was issued and must remain posted for 30 days. |
