25 | On 03/08/2024, case management visit is conducted by Licensing Program Analyst (LPA) Lady Cabrera and met with Site Supervisor Rosa Rabago. LPA met with Rosa at the facility’s office. Director Mary Ann Mooney, Program Administrator Letisha Brooks and Director of Head Start Yolanda Gonzales were present via Microsoft Teams Meeting.
On 01/25/2024, staff notified Site Supervisor of a teacher mishandling a child on 01/23/2024. On 01/30/2024, LPA arrived at the facility to investigate a complaint, it was confirmed the facility did not report the incident to Fresno Community Care Licensing (CCL) by telephone or fax within the CCL’s next working day and during its normal business hours. LPA provided Reporting Requirements regulation to Site Supervisor and the Director.
On 01/30/2024, Director Maryann Mooney reported the incident to CCL by telephone. On 01/31/2024, an Unusual Incident Report was submitted to the CCL.
During a complaint investigation, LPA discovered another similar incident occurred on 11/23/2023 where the same teacher mishandled a child in the outdoor play area. Per records reviewed and interviews, CCL did not receive a phone call and/or an Unusual Incident Report. Facility did not comply with the reporting requirements regulation.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited. Site Supervisor Rosa Rabago was provided a copy of their appeal rights.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
