25 | Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced case management–incident inspection. LPA met with Program Manager Rebecca Quintanilla, took a census, and toured the indoor and outdoor areas. The purpose of the inspection was to follow up on an unusual incident involving an injury that occurred on 08/18/2026.
On 08/18/2026 at approximately 10:45 a.m., during outdoor play, two children collided while running near the play structure. As a result, one child fell forward and made contact with part of the structure, sustaining minor swelling and a small cut. Staff responded immediately, provided first aid, and escorted the child to the onsite clinic. The authorized representative was notified, and the clinic doctor assessed the child, confirming there was no injury to the eye and no signs of concussion. The child was released to the authorized representative and remained out of care until 08/24/2026.
Record review conducted today confirmed the child was signed in and out on 08/18/2026 from 7:44 a.m. to 11:00 a.m. Records also verified that the child returned to care on 08/24/2026 and was observed present today.
LPA conducted interviews with staff, reviewed facility records, and inspected the outdoor playground. Staff reported that required supervision practices, including zoning and scanning, were in place during outdoor activities and that safety expectations are reviewed with children prior to outdoor play. Staff stated they responded promptly to the incident by providing first aid and notifying the authorized representative. During the inspection, LPA observed no hazardous conditions on the playground.
Based on today’s inspection, staff interviews, record review, and observations, the incident appears isolated. Staff responded appropriately and followed required procedures. No supervision concerns or hazards were identified.
An exit interview was conducted, and the report was reviewed with Program Manager Rebecca Quintanilla. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and must remain posted for 30 days.
