On March 21, 2024 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced plan of correction (POC) inspection. LPA McWilliams met with Director Scott Elliott and informed him the purpose of the inspection. A tour inside and out of the facility was conducted and a census was taken.

On January 24, 2024 LPA McWilliams conducted an unannounced annual inspection and observed through file reviews that the facility did not have complete staff files. LPA McWilliams conducted an initial plan of correction for the staff files on February 26, 2024 and observed that staff files were still not complete. LPA McWilliams went to the school district office on that day and met with the school district personnel and went over all of the forms that need to be accessible to Community Care Licensing Division during inspections.

During the POC inspection on this date LPA McWilliams thoroughly went through staff files and observed that all staff files are complete.

Exit interview conducted and report was reviewed with Director Scott Elliott. Appeal rights were provided.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
