25 | On this date Licensing Program Analyst (LPA) Adrian Pizano conducted an unannounced Case Management inspection. LPA met with Teacher Ana Villanueva, toured the facility, and took a census. The purpose of today’s inspection was to discuss an incident that was reported to Community Care Licensing (CCL) on 11/14/2023.
Teacher stated that on 11/13/2023, Child 1 was running up the stairs of the play structure to access the top f the slide when he tripped and fell. During the fall, Child 1 fell face first and struck Child 1’s mouth on the stair. First aid was administered and Child 1’s parent was notified. Parent sought medical attention for the child and one tooth had to be extracted. Child 1 was released and returned to care on 11/15/2023. LPA discussed supervision at the time of the incident, and it was determined that appropriate supervision was in place. Staff will continue to remind children to be careful and not run up the stairs or on the play structure. LPA inspected the area where the incident took place and did not observe any safety hazards or areas of concern.
This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements.
No deficiencies cited during today’s inspection.
