On 3/21/22 an unannounced complaint inspection was conducted today by Licensing Program Analyst (LPA) Caroline Harris. LPA met with Director, Heidi Brown and toured the facility inside and outside. A census was taken. LPA reviewed the above listed allegation with the director. The purpose of today’s visit was to open and investigate the above complaint allegation. The investigation consisted of interviews with the director, Heidi Brown, owner, Alexis Savedra along with classroom observations. Copies of facility records including children's files, and roster were also reviewed. Sign in sign out forms are electronic. This facility does not have video surveillance.

Although the allegation may have happened or is valid, based on information obtained during the investigation, there is not a preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is unsubstantiated.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, are being cited on the attached LIC 9099D. An exit interview was conducted with Director, Heidi Brown. A copy of this report along with appeal rights were discussed and given to Heidi Brown. A Notice of Site Visit is to be posted for 30 days.

Daily Inspection for Illness. The director agrees to implement written inspection procedures on conducting daily physical and illness inspections. Also, the director agrees to train staff on new daily inspection procedures.

accepted. After the child has been determined to be without obvious signs of illness and has been accepted, the center shall require that the person sign the child in. This requirement was not met as evidenced by the center director admitting that they are not conducting daily physical inspections of the children, looking for scratches, bruise's, etc. nor is there any procedure in place or being documented. This poses a potential risk to the health, safety, or personal rights of children in care.

The director will submit a copy of the training agenda and staff sign-in sheet along with a copy of the daily inspection form to the Fresno CCL office by the due date of 4/4/22.
