25 | On this date, Licensing Program Analyst (LPA) Lady Cabrera met with Director Raquel Hernandez for an unannounced case management inspection. LPA toured the facility, and a census was taken. An Unusual Incident Report was submitted to the Fresno Community Care Licensing Office (CCL) regarding an incident that occurred on 01/23/2024, where a day-care Infant 1 fell, and hit the back of their head.
At the time of the incident, there were two staff, and four infants present. Two infants were sitting down at the table eating a snack and the two older infants (Infant 1 and Infant 2) were playing together. Teacher 1 was supervising the smaller infants as they finished their snack. Teacher 2 was cleaning the toys on the floor next to the carpeted area in the center of the classroom, about one to two feet from Infant 1 and Infant 2. Infant 1 and Infant 2 ran in circles with each other and their stomachs collided with one another. Infant 1 hit the back of their head on the carpeted area, while Infant 2 fell on their bottom. Infant 1 started crying while Infant 2 continued running on the carpeted area.
Teacher 2 comforted Infant 1 on the staff’s lap and kept Infant 1 under close observation. Shortly, Teacher 2 observed that Infant 1 had their fist clenched, infant’s body became stiff, and their eyes roll back for a few seconds. Teacher 2 immediately placed Infant 1 in the recovery position. Director Raquel Hernandez contacted 911 and notified the infant’s parents. Ambulance responded and transported Infant 1 to the hospital for further evaluation. The parent reported CT scan results revealed no signs of head injury and had no bump on the infant’s head. Emergency doctor reported infant experienced “breath holding spell” and had no signs of concussion or injury. No suspicion of seizure. The following day, the parents attended a pediatrician appointment. The pediatrician reported the infant did not experience a seizure and believed it was a mild concussion that it did not show up in the CT scan. As of today, there has been no other incidents at the facility regarding Infant 1.
During today’s visit, LPA observed the carpeted area in the center of the classroom where the incident took place and determined that appropriate supervision and ratio was in place.
