On 12/12/2023 Licensing Program Analysts (LPAs) Claribel Soto and Nancy Her, conducted an unannounced case management Inspection. LPAs met with Director Gloribel Magana and toured the facility indoors and outdoors and a census was taken. The purpose of the inspection was to follow up on the deficiency cited on 07/19/2023 to inspect the wall that was installed to separate the indoor activity space from the napping area. Upon inspection a permanent wall has been added that meets Title 22 regulations to separate the indoor activity space from the napping area. As a result, the indoor activity space square footage is approximately 241 which does not accommodate licensed capacity of 9.

Exit interview conducted and report was reviewed with facility representative Gloribel Magana.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
