On 09/17/2024, Licensing Program Analyst (LPA) Justeene Tamayo conducted an unannounced Required 1-Year inspection at the Chairenza Family Child Care. Upon arrival, the LPA met with the licensee, Nancy Chairenza who guided the LPA on a tour of the facility. Family members residing in the home include 3 adults (licensee, licensee spouse, licensee adult daughter) and one child. Per LIS, facility annual fees are current. Per the licensee, Days/Hours of Operation are Tuesday through Friday, 7 AM to 6 PM. There were 4 preschool children, and 1 school age child in care. Incidental Medical Services (IMS) were discussed.

The Home is set up as follows:
This is a two story 4 bedroom, 3 bathroom home with kitchen/dining, family room, living room, formal dining room, enclosed patio-sunroom/play room (permitted), laundry room and garage. There is an in-ground pool on the premises. The garage is used for storage only and no childcare activities are conducted there. The home was inspected for safety, comfort, cleanliness, telephone service, central air, and heat and ventilation. The home has central heating and air conditioning. All windows have screens and are free of cracks, bugs, and debris.
Ø Main Area: Main care is provided in the enclosed patio-sunroom/playroom. There are two bathrooms for child care. Children use the bathroom in the enclosed patio playroom-sunroom and in hallway next to the kitchen. Children eat, play, conduct activities, and sleep in the enclosed patio-sunroom/playroom.
· Patio-Sunroom/Playroom: LPA observed age-appropriate toys and furniture for the children. LPA observed: A small table was observed with a total of eight chairs. Several plastic storage bins were observed in which games and toys are stored for the children. A small play kitchen was located by the door with which children can play. There are games and books on the premises of this facility. There are mats on the floor that have educational/learning activities on them. The mats were observed to be in good condition.

· Children Bathroom (#1 and 2): Children use the bathroom in the enclosed patio playroom-sunroom and in hallway next to the kitchen. The Bathroom was toured and inspected sink/toilet is in operable condition. Toilet and faucets are clean, safe, and operable. All poison and medications are made inaccessible to children with child safety latches on the sink cabinet and drawers. The bathroom was observed to be free and clear of hazardous items. The bathroom was clean, sanitized, and in good repair.
· Backyard: The backyard was inspected; The backyard is gated all around. The outdoor play area was observed to be free of hazards, and loose and sharp parts. Children play in the backyard. The backyard is completely fenced. There is an additional property next door that is used for outside play along with the backyard of the home. The additional yard/property next door has a jungle gym with a slide and swing that is anchored, a grassy area and other Little Tikes play equipment. The backyard of the home has a dog run area, grassy area, patio, and in-ground pool. The in-ground pool has mesh fencing and gate. The gate is self-closing and self-latching and has 5 feet fence/gate in height. The pool is in regulation. LPA reminded licensee, barbecue pits must be covered and in inaccessible area.
Ø Off-limit: Off-limit areas include the entire upstairs (Bedroom #2, #3, and #4, bathroom #1 and #2), office/Bedroom #1, laundry, and garage (key lock), and the Kitchen/Dining Room.
Ø Others:
· AC/Heating Unit was observed. AC/Heating Unit is located on the right side of the home and is inaccessible to children via barrels blocking access to the unit.
· Bodies of water:
LPA observed the in ground swimming pool to be fully barricaded by mesh fencing. The mesh fence is at least five feet high and the fence does not obscure the pool from view. The gates swing away from the pool, self-close, and have a self-latching device located no more than six inches from the top of the gate.
· Electrical outlets: All unused electrical outlets are plugged in and made inaccessible to children.
· Fire extinguisher (2A10BC): LPA observed there is a required fire extinguisher (2A10BC) fully charge and located in the playroom bathroom. It meets standards established by the State Fire Marshall.
· Fireplace: The fireplace was observed in the off-limits Family room and is screened.
· Hanging window blind cords: The cords are accessible to children.

Incidental Medical Services (IMS): Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to the publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The Licensee will be providing IMS to the children at this time.
· Isolation area (Illness): Per the licensee, if the child shows signs of illness, he/she will be separated from other children and stay in Living Room.
· Medications and cleaning solutions: (upper kitchen cabinet, laundry room), medicines (off-limits bedroom), and hazardous items (sharp knives in the kitchen unreachable to day care children)
· Napping: Children will nap in the designated nap areas with adult supervision. LPA observed 8 mats in the closet.
· Pets: 1 dog. They have current vaccinations.
· Phone service: There is a working landline or cell phone
· Smoke Detectors and Carbon Monoxide: The smoke detectors and carbon monoxide devices tested operable.
· The First Aid kit is located in the key-locked closet inaccessible to children. The First Aid Kit was observed complete with supplies and a first aid manual.
· Transportation: The licensee will not transport daycare children.
· Weapons or Firearms: Per the licensee, there are firearms at the facility at this time. Firearms are kept in a gun safe in the upstairs off limit bedroom separate from the ammunition (kept upstairs, locked). LPA confirmed that ammunition is kept separately. Firing pins are stored and locked separately from firearms.

Ø Documentation:
· CPR/First Aid: LPA observed licensee has current Pediatric CPR and First Aid Training with expiration date (01/20/2026). Mandated Reporter expires on 01/02/2025.

· Facility Roster: LPA observed Child Care, Facility Roster is not update. Per Licensing Information System, facility annual fees were current.
· Fire Drill and Disaster Drill: Per the licensee, fire and disaster drills are conducted every 6 months; the last fire/disaster drill was conducted on 09/17/24.
· Immunization: The licensee has the required immunization's. The licensee provided a written statement declining the influenza vaccination.
· Infant Sleeping Plan (LIC 9227): No Infant enroll in the program.
· Licensee has posted as required the Facility License, Emergency Disaster plan, and Parents Rights Poster.
· Mandated Reporter Training: The licensee and her assistant have not completed the online mandated reporter training at www.mandatedreporterca.com

Ø The following information was discussed with the licensee:
ü Mandatory Forms for the children’s files and provider’s files.
ü Requirements for fire drills, earthquake drills, and documentation for both.
ü The role and responsibilities of being a mandated reporter were discussed.
ü The licensee is reminded that 100% supervision is required for children at all times.
ü Capacity requirements, Roster requirements, Posting requirements, and Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Awareness. The role and responsibilities of being a mandated reporter were reviewed. The licensee was reminded that supervision is always required for children in care.
ü The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B

No deficiencies have been cited at this time. An exit interview was conducted, and a copy of this report was provided to licensee, along with a copy of her appeal rights and Notice of Site Visit.
