On 8/12/2025, Licensing Program Analyst (LPA) Ortega conducted an unannounced Required 1-Year inspection at the Juarez Family Child Care Home. Upon arrival, the LPA met with licensee, Yolanda Juarez who guided LPA on a tour of the facility. All adults living in the house have been background cleared. Per the Licensee, hours of operation are a day Monday through Saturday 5am to 5pm. Per licensee, no overnight care is provided at the moment. Upon arrival, LPA observed 7-day care children in present and the 4 adults providing care and supervision. Licensee is within the ratio for a large facility.

Physical Plant:
This is a one-story, 5-bedrooms, 2 ½ bathrooms home with a kitchen, living room, dining area, backyard, garage. The daycare rooms are separate from the rest of the house. The home was inspected for safety, comfort, cleanliness, telephone service, central air, and heat and ventilation. The house has central heating and air conditioning. All electrical outlets are covered.

Main Care Area: Main care is provided in the 2 rooms that are separate from the main house by a door remained locked and with a child safety door knob. Children use the ½ bathroom located in one of the day care rooms. LPA observed age-appropriate toys and furniture for the children. There are age-appropriate games and books on the premises. Per licensee, there is a designated area for ill/sick children in the playroom (day care area) near the main entrance. Children nap in the playroom in mats

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children's bathroom: Children use the ½ bathroom located in the playroom. The bathroom was clean, sanitized, and in good repair. The bathroom was toured and inspected sink and toilet which is in operable condition.

Kitchen/Dining Room: The kitchen is made inaccessible by a doorknob that separates the playrooms from the rest of the house. Sharp items are made inaccessible to children. Cleaning compounds are stored in a lower cabinet, made inaccessible by door safety knob on door that gives access to the rest of the house. Per licensee, she does currently have a food program. Meals offered: Breakfast, AM snack, Lunch, PM Snack and Dinner.

Backyard/Outdoor areas: The backyard is completely fenced in with cement block. The backyard has play equipment that is safe and in well repair. The backyard was found to be clean, free of debris or harmful materials. There are two storage sheds that remained locked. Per licensee and LPA’s observations, there are no bodies of water in the premises; however, a hot tub is being built in a separate unit outside the day care area. The unit remains locked at all times. There are 3 pets in the home.

Off-limits: Off-limit areas include all the bedrooms (3)of the home, bathrooms #1 and #2, and storage shed. The laundry maintained locked.



Care and Supervision: Licensee is within the ratio for a large facility. The Licensee was reminded that supervision is always required for children in care.

Mandated Reporter training certificate is dated 6/24/2025 for both Licensee.

Fire/Health/Safety: There is a cell phone kept charged and on the Licensee at all times.
Smoke Detectors and Carbon Monoxide were observed to be in operable conditions.
The First Aid kit was observed to be complete with supplies including thermometer, tweezers, scissors, gauze, bandages, cleansing pad/solution, and a first aid manual. LPA observed a required fire extinguisher (2A40BC) reading in Green purchased from Home Depot on 7/27/2025. Per licensee, she currently does offer transportation, LPA observed current car registration, insurance and driver’s license.
Medications/ Hazardous Materials: Medications are stored in the master bedroom, made off-limits and secured with a safety doorknob. Per licensee, there are No Firearms at the facility. LPA did not observe any firearms during the inspection.

Incidental Medical Services (IMS): Incidental Medical Services (IMS) policy was discussed. For IMS information, see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The Licensee does not have any IMS children at this time.

Records: Children’s records were observed to be complete and current.
Licensee’s CPR/First Aid is maintained current, it expires on 04/02/24. Mandated reporter is maintained current, it expires 06/14/2027. Per Licensee, fire and disaster drills are conducted every 6 months; the last drill was documented and conducted on 07/25/2025 at 11:00 a.m. Per LPA’s observations, the Facility License, Emergency Disaster plan, Earthquake Preparedness and Parents Rights Poster were posted.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care.

Licensee Juarez was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee Juarez and discussed the Child Care Licensing Safe Sleep webpage at www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Inspection was conducted in Spanish; LPA is certified bilingual by the department.

No deficiencies have been cited at this time. A notice of site visit was provided to licensee and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.Exit interview conducted and report was reviewed with the licensees, along with her appeal rights and Notice of Site Visit.
