32 | LPA observed the dining table to be covered with old food wrappers, fast food condiments, old water bottles, dirty plates, plastic bags, storage containers, various paperwork, along with sticky food and drink stains on top of the table. LPA observed dining chairs stacked with items including boxes of instant noodles, a food cooler bag, storage containers, reams of paper, along with articles of clothing. LPA also observed the kitchen area to be cluttered with food containers, and old water bottles.
Based upon LPA’s observations, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated.
Per Title 22, Division 12, of the California Code of Regulations, the following deficiency is being cited: (see next page).
A copy of this report and Appeal Rights were provided and an exit interview was conducted with Licensee, Hilda Albayeros.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
