On 08/03/2023, An unannounced complaint inspection was conducted today by Licensing Program Analyst (LPA), Jose Penate. LPA met with Director, Jessica Okamoto. The purpose of the visit is to open and close a complaint investigation.

With the allegation of, Staff did not adequately supervise day care child in care, was investigated. LPA interviewed staff at the facility and during interview it was stated that the facility did allow a child in care to utilize the staff restroom located in the director’s office and the door being closed. Director stated that the child was yelling and screaming due to needing assistance in wiping. Director stated that she was in the same room but not in the restroom with the child in care.

This agency has investigated the complaint with the allegation of, Staff did not adequately supervise day care child in care, and based on interview the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED.

Per California Code of Regulations, Title 22, Division 12, deficiency cited (see LIC 9099-D).

An exit interview was conducted with Director, Jessica Okamoto.

A printed copy of this report as well as appeal rights were provided Jessica Okamoto at the conclusion of the visit.

A Notice of Site Visit Form was posted on parent's board and must remain posted for 30 days.
