13 | On 03/11/26, Licensing Program Analyst (LPA) Sonja Navarrette conducted an unannounced complaint inspection to provide findings. LPA met with Facility Representative, Vanessa Garcia who accompanied LPA during tour of facility both inside and outside. LPA explained the allegations and a census was taken. LPA interviewed witnesses, parents, reviewed sign-in/out sheets, and facility records. During the investigation, LPA reviewed records, interviewed staff, witnesses and parents who provided inconsistent statements. Although a child sustained an unexplained injury, there were inconsistent statements as to the cause of the injury. In addition, there were inconsistent statements regarding how staff were providing inadequate supervision and failing to follow reporting requirements.
The investigation revealed through interviews, LPA’s observations, and review of records, that although the above allegations may have happened or are valid, there is not a preponderance of evidence at this time to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
