25 | On 3/18/2026, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced case management–incident inspection. LPA met with Site Supervisor Gensis Ramos. LPA toured the facility inside and outside, and a census was taken. The purpose of today’s inspection was to follow up on an unusual incident report received by the Fresno Regional Office on 2/26/2026.
On 2/26/2026, at approximately 10:09 a.m., a child C#1 was climbing on a play structure when she slipped and hit the back of her head. Staff assisted the child, observed her to ensure she was responsive and alert, and conducted a head injury checklist. Staff provided an ice pack and seated her at a nearby table, where another head injury checklist was completed. While one staff member assisted with the checklist, another assisted with the ice pack, and a third staff member contacted the parent. The parent arrived at the facility at approximately 10:35 a.m. and took child.
During today’s inspection, LPA conducted interviews with staff, reviewed staff and children’s files, and obtained facility documents. After gathering information, LPA determined that staff took appropriate action during the incident and per Site Supervisor Gensis Ramos a meeting was held to discuss safety rules during outdoor play. Additionally, staff and children review outdoor rules prior to outdoor play time. This appears to be an isolated incident.
An exit interview was conducted, and the report was reviewed with Site Supervisor Gensis Ramos. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
