On 01/27/2025, Licensing Program Analyst (LPA) Lady Cabrera, conducted an unannounced Annual Random Required Inspection for the preschool license. LPA toured the facility indoors and outdoors and a census was taken. This program operates year-round. This facility provides full-time care Monday through Friday from 7:00 AM to 5:30 PM. This facility is on the campus of Planz Elementary School. This program will operate in classrooms TK1 & TK2.

All poisons are kept in a locked storage area Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. There is no swimming pool or other bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Areas around high climbing equipment and slides have cushioning material to absorb falls. All toilets and handwashing facilities are in safe and sanitary condition. Facility has one or more functioning carbon monoxide detectors. Floors in the facility are clean and safe. Solid waste storage containers have tight-fitting covers and are in good repair.

Drinking water is available both indoors and outdoors. Drinking water is provided by personal water bottles, drinking fountains and water bottles This facility does provide meals to children, snacks are provided. The meals are prepped in the school cafeteria, then transported to the classrooms to be served. Menus are posted at least one week in advance where an authorized representative can view them.

Capacity and limitations as specified on the license are being maintained. At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department.

Based on record review and interview, Kitchen Stove identified as D, TK1/TK2 Exterior Drinking Fountain identified as E and F showed to have been impacted and results indicates out of compliance over (ALE) 5.5 ppb, which poses a potential health, safety or personal rights risk to persons in care.

Licensee stated parents and guardians were notified of the ALE and will post ALE results at a reasonable location for Authorized Representatives to review. Outlet D, TK1/TK2 Drinking Fountains will be replace/repair and is in the process of being retested. Outlet D, TK1/TK2 Drinking Fountains were verified to be made temporarily inoperable.
A new lab report along with LIC 9275, LIC 9276, facility sketch and any supporting photos will be submitted to the Department by POC date 02/27/2025.

All children are under supervision, including visual supervision, of a teacher at all times. Facility meets teacher-child ratios. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training.

LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to resources such as forms, regulations Provider Information Notices (PINs), and Quarterly Updates.

LPA discussed Reporting Requirements as outlined in the regulations (Section 101212).

Criminal record clearance records are completed as condition for employment and maintained by the Geenfield Union School District Office.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA referred Director to the Department website for lead: www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information.

CCC COMPLETED TESTING AND IS IN THE PROCESS OF REMEDIATING LEAD EXCEEDANCES.



Fresno Regional Office received notification of water lead exceedance at the facility resulting from a recent water lead sampling test. Based on records reviewed from the Child Care Center Lead Sampling and Reporting Tool, it was determined that multiple outlets were tested on 03/23/2024 and retested on 05/25/2024. On 05/25/2024, Kitchen Stove D was impacted with an ALE of 14.8 ppb and TK1/TK2 Exterior Drinking Fountain identified as E and F were impacted with an ALE of 5.93 ppb and 11.9 ppb. These results indicate out of compliance over (ALE) 5.5 ppb. (See Plan of correction for further corrective action on LIC809D). Kitchen Stove D and TK1/TK2 Exterior Drinking Fountain E and F has been temporarily made inoperable and is in the planning stages of replace/repair Licensee agrees to continue with the resampling process for an action level result of under 5.5 ppb for compliance with a Certified Sampler.

Children drink water bottles. Water is obtained from the Classroom K1 Bubbler and Classroom TK2-Bubbler which was also tested for lead and did not result in any ALE. LPA reviewed the requirements to post results and notify parents of ALE. On 04/25/2024, Licensee submitted the LIC 9275 External Water Sample Self-Certification Form, LIC 9276 Child Care Center Sampling Checklist, the facility sketch with identified outlets that were tested with photos that were taken during testing.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
