25 | On 10/30/24, Licensing Program Analyst (LPA), Denisia Jimenez and Licensing Program Manager (LPM), Scott Herring, met with Applicant, Osama Albawab for a pre-licensing inspection. Applicant and 2 minor children reside in the home. Background clearances are discussed, and LIS 531 is signed indicating that the adults currently living in the home and/or providing care and supervision to children have a criminal record clearance. Fire clearance was granted on 10/22/24.
Facility was inspected inside and outside as shown on the facility sketch and the following items were discussed:
· Fire clearance was received on 10/23/24. Fire pull alarm is located in the home’s great room (daycare room).
· This is a single story, 5 bedrooms and 3 bathrooms home and children will have access to the kitchen, great room, dining room, all 3 bathrooms, all bedrooms except bedroom #5, and backyard. Off-limits rooms/closets are made inaccessible by use of safety locks.
· There is an electric fireplace in the owner’s suite that applicant states will not be used during day-care hours. There is central air heating/cooling ventilation for safety and comfort.
· LPA observed in the great room (daycare room), little tykes children’s table for the children. LPA also observed in bedroom #2, 2 playpens, 3 booster seats, and cubbies in the closet. Children will nap in the bedrooms and great room (daycare room). Applicant understands he is to supervise children at all times. LPA provided applicant with Individual Sleeping Plan and Safe Sleep handout.
· Facility has 1 fire extinguisher located in the hallway. There is a functioning smoke alarm, carbon monoxide alarm and first aid kit in place.
· Applicant’s Pediatric CPR and First Aid certification was completed through EMS Safety with Emergency Medical Services Authority stickers (EMSA) and expires on 04/2025. Applicant stated he has 2 minor children 14 and older that will assist in the day care and applicant understands that minor assistants cannot be left alone with the day care children.
