On 10/30/24, Licensing Program Analyst (LPA), Denisia Jimenez and Licensing Program Manager (LPM), Scott Herring, met with Applicant, Stephen Cutty for a pre-licensing/ change of location inspection. Applicant, and 1 minor child reside in the home. Background clearances are discussed, and LIS 531 is signed indicating that the adults currently living in the home and/or providing care and supervision to children have a criminal record clearance. Fire clearance was granted on 10/18/24.

Facility was inspected inside and outside as shown on the facility sketch and the following items were discussed:
· Fire clearance was received on 10/21/24. Fire pull alarm is located on the home’s living room.
· This is a two story home. There is a gate at the bottom of the stairs and a gate at the top of the basement. Care and supervision will be provided throughout the whole house and backyard, except for the furnace room that is in the basement. Off-limit furnace room/closets are made inaccessible by use of door locks.
· There is a fireplace in the living room that applicant states will not be used during day-care hours. The fireplace is covered by a black plywood. There is central air heating/cooling ventilation for safety and comfort.
· LPA observed in the dining room, 2 children’s tables with 12 chairs for the children. Licensee stated he has not brought over the toys/play equipment because they are currently at the other home. Children will nap in living room and downstairs bedroom. Applicant understands he is to supervise children at all times. LPA did not provide applicant with Individual Sleeping Plan and Safe Sleep handout. Applicant has the Individual Sleeping Plan and Safe Sleep handout.
· Facility has 2 2A10BC fire extinguishers located in the living room and laundry room. There is a functioning smoke alarm, carbon monoxide alarm, AED device, and first aid kit in place.

· Applicant’s Pediatric CPR and First Aid certification was completed through EMS Safety with Emergency Medical Services Authority stickers (EMSA) and expires on 06/30/2025. Applicant’s assistant will be Emily Prieto and completed Pediatric CPR and First Aid certification through EMS Safety with Emergency Medical Services Authority stickers (EMSA) and expires on 06/30/2025.
· Preventative Health and Safety with Nutrition and Prevention of Lead exposure certification was completed on 08/25/2024.
· Applicant completed the Mandated Reporter Training on 10/22/24. Applicant’s assistant training was completed on 06/01/2023.
· Knives will be stored in the top kitchen cabinet and above the stove on a magnetic strip. Medications will be stored in the living room in a locked box inside a locker. Cleaning compounds are stored in the laundry room locked closet.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. Applicant states he does not need a sample of a fire drill log.
· Applicant is advised at least one staff member with current training in pediatric first aid and pediatric CPR is to be on site at all times children are present.
· There are no bodies of water in the home or premises.
· Applicant states there are no pets in the home or on the premises.
· Applicant states there are no firearms, ammunition or poisons in the home or premises.
· Applicant is reminded that any advertising (of day-care) such as business cards, flyers/posters, and/or signs must include facility number as per Title 22 Regulation "Advertisements and License Number" 102359 (a).
· Applicant is advised that smoking is prohibited on the premises of a family child care home as specified in Health and Safety Code Section 1596.795(a). Applicant states the home is smoke-free.
· Applicant states he will be transporting day care children. Applicant understands that he must have proper car restraints and/or car seats for all the children under his care when transporting children.
· Fenced backyard has equipped toys and appropriate play equipment for the children. There is a 8x14 game room with a tv and a Pacman gaming.
· LPA discussed safe sleep pending regulations and Safe Sleep Regulation Concepts.

· Applicant is advised it is his responsibility to read and maintain his facility incompliance with Title 22 Regulations. Title 22 Regulations can be found at www.ccld.ca.gov.
· SB 792 immunization's are on file.
· Applicant is advised Fresno Community Care Licensing Department has inspection authority and can inspect all rooms in the home, garages and/or separate dwellings on the premises.

· Incidental Medical Services (IMS) policy was discussed. Incidental Medical Services (IMS) are not currently being provided. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383.

Required postings, forms packet, which included Parent Notification Requirement and LIC9224 were not provided. Applicant has all the required postings and licensing forms. Applicant is advised that he may access CCLD website at www.ccld.ca.gov for additional forms and licensing updates. Applicant is also reminded that it is his responsibility to read the regulations periodically. Applicant states he will operate his day care Monday through Sunday 24 hours a day and as arranged. "Overnight Care" means care being provided to children anytime between the hours of 6 p.m. and 6 a.m. Care provided during the day and overnight combined shall not exceed 24 hours from the time the child entered into care.

LPA & applicant discussed the Community Care Licensing website: LPA and applicant discussed new additions to the website that include the new PIN (Provider Information Notification) and information for providers including the Quarterly Update that informs licensees of new legislation and regulations. Please follow these steps go to http://www.cdss.ca.gov/, click on “information and resources” click “Community Care Licensing” Click “quarterly updates” click “Child Care advocates program” and register to PIN.



Pending a final review of application file, licensure as a Large Family Day Care Home capacity of 14 children ages under 18 years will be recommended effective 10/30/24.
