On 1/30/2024 at 9:50am, Licensing Program Analyst (LPA) Carol Fowler conducted an unannounced 1-Year Required inspection. LPA met with Imelda Caregiver, and explained the purpose of the visit. Taralyn Kawata, Director of Services arrived at 11:15am and Administrator arrived at 1:03pm. The Administrator currently holds a certificate (#6067674740) that expires on 11/17/2025. The facility’s fire clearance was approved for fourteen (14) non-ambulatory residents.

LPA toured the facility with Caregiver including but not limited to bedrooms, bathrooms, kitchen, common area, and backyard. The facility consists of ten (10) total bedrooms, and four (4) bathrooms. No bodies of water observed. A comfortable temperature is maintained at 72 degrees Fahrenheit. LPA observed lighting in all rooms are adequate for the comfort and safety of the residents. The hot water temperature in the residents’ shared bathroom was measured at 99.4 degrees Fahrenheit. Residents’ bathrooms are equipped with grab bars.

Smoke detectors and carbon monoxide were in operating condition during visit. Fire extinguisher was last serviced on 07/26/2023. Emergency Disaster Plan is posted. First aid kit was observed to be complete.

LPA reviewed 5 staff files which were all incomplete. LPA also reviewed 5 resident files which were complete.

LPA observed the following deficiencies:



· At 10:43am, LPA observed S2 has not been fingerprint cleared and not associated to the facility.
· At 10:46am, LPA observed unlocked scissors in resident bathroom.
· At 10:50am, LPA observed arm&Hammer oxy clean, Ajax, pledge, drano, purell disinfectant, rust lime calcium, disinfectant spray Q, fabulso, d-molish.
· At 10:58am, LPA observed in room A ibuprofen, room B, miralax, tylenol and scissors room G scissors.
· At 11:00am, LPA observed unlocked located in the living room glue and spray paint, .
· At 11:18am, LPA observed pick, rake, box and small gardening tools.
· At 11:21am, LPA observed a mattress, box spring, bed frame, 2 walkers, 2 ladders and wood planks located on the side and front yard.
· At 12:00pm, LPA observed 5 staff files were incomplete.
· At 12:20pm, LPA observed facility last emergency drill was conducted 3/22/2023.
