13 | On 9/25/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced subsequent complaint visit to conduct additional interviews and deliver findings. LPA introduced self and stated purpose of visit. LPA met with Executive Director/Administrator, Jeffrey Toomer to conduct complaint visit.
LPA conducted facility tour, conducted interviews, and gathered documentation during complaint visit. During record review it showed that at time of assessment for R1, the additional service for showering or bath was requested by R1 and their responsible party. The day after admission, the responsible party requested the service to be removed. Per physician report submitted at time of admission, R1 was able to complete this activity of daily living independently. Records indicate the additional service was removed from billing.
This Department has found that the above allegations are UNFOUNDED, meaning they were false, could not have happened, and/or were without reasonable basis. We have therefore dismissed the complaint.
