On 9/26/25 at 12:30 pm Licensing Program Analyst (LPA) J. Leffall conducted a subsequent complaint visit to deliver findings on above allegation. LPA met with House Manager (HM) Jethro Pandy and stated purpose of visit.

The department conducted interviews and reviewed records. Based on the information obtained, the above allegation is unsubstantiated.

No deficiencies issued.

Exit interviewed conducted. A copy of this report was provided to Administrator whose signature confirms receipt of the report.
