Licensing Program Analyst (LPA) Shawna Doucette arrived unannounced to conduct an initial 10-day complaint inspection. LPA conducted an investigation and delivered findings. LPA explained reason for the visit and met with Administrator Ramona Eleco.

LPA requested copies of C1's file. LPA interviewed residents and staff. LPA requested copies of staff schedule, resident roster that includes incontinent and non ambulatory clients in care.

LPA requested the record of the signal system showing when signal system was pulled and response times. Administrator stated the facility signal system does not record this information. LPA checked the signal system and a light will blink outside of the clients door. Once the pull chord is pulled the office staff are notified.
After reviewing records and conducting interviews, facility has one caregiver assigned per hall. After conducting interviews, staff did not responded for over an hour to a client needing assistance.

Based on record review and interviews, the preponderance of evidence standard has been met; therefore, the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, Title 22, Division 6, Chapter 8, a deficiency is being cited on the attached 9099-D.

An exit interview was conducted with Administrator Ramona Eleco and a copy of this report along with appeal rights and plan of correction were provided.

87411 Personnel Requirements - General (a) Facility personnel shall at all times be sufficient in numbers, and competent to provide the services necessary to meet resident needs. In facilities licensed for sixteen or more, sufficient support staff shall be employed to ensure provision of personal assistance and care as required in Section 87608, Postural Supports. Additional staff shall be employed as necessary to perform office work, cooking, house cleaning, laundering, and maintenance of buildings, equipment and grounds. The licensing agency may require any facility to provide additional staff whenever it determines through documentation that the needs of the particular residents, the extent of services provided, or the physical arrangements of the facility require such additional staff for the provision of adequate services.
