Community Care Licensing
Resident, medication and staff records were reviewed. LPA observed medication errors for R1. R1 medication count did not match start date on centrally stored log. Start dates were pre logged for 10/18/24. R1's medication was instructed to be given twice a day and facility was only administering one time per day. Facility was administering medications with no prescription labels. LPA took photos of medications.

Current first aid and CPR were reviewed. Administrator does not have a current Administrator Certificate. LPA's checked the pending list and Administrator's certificate does not show pending. Administrator was not able to provide personnel file, stating it was lost.
Residents in Bedroom 1 and in Bedroom 2 had full bed rails without a physician order.
LPA's reviewed facility Plan of Operation which did not contain a plan for bedridden residents in care. LPA Doucette requested Licensee to submit complete current plan of operation.
Refer to 809d for deficiencies. Civil Penalties were issued for repeat violations.
A copy of this report with plan of correction and appeal rights were provided to the Administrator.
