Licensing Program Analyst (LPA) at the unannounced to conduct a complaint investigation. LPA was granted entry by Administrator Voncille McDaniel

LPA's interviewed staff and client.

Based on interviews, facility staff have called C1 by a nickname, however C1 is not offended by the nickname. Interviews revealed staff treat C1 with dignity and respect.

Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.
