On 05/27/26, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an initial complaint investigation and deliver complaint findings on the above allegation. LPA met with Administrator Wendy Lee and stated the purpose of the visit.

During the course of the investigation, records were reviewed, facility was toured, and interviews were conducted. Due to the lack of supervision, R2 has wandering behavior and had wandered into R1’s room without supervision leading to a physical altercation where R2 was sent to the hospital for medical treatment.

Based on interviews conducted and records reviewed, the preponderance of evidence standard has not been met, therefore, the above allegation is found to be SUBTANTIATED. Exit interview was conducted. A copy of this report and appeal rights was provided to Administrator, whose signature on this form confirms receipt of this report.
