On 06/26/2026, Licensing Program Analyst (LPA) J. Duarte met with Administrator Crystianna Robinson to deliver findings for the above allegations.

The Department conducted interviews and reviewed records. Based on the interviews conducted and records reviewed, the allegations are unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are Unsubstantiated.

An exit interview was conducted. A copy of this report was discussed and provided to Administrator, Cystianna Robinson, whose signature on this form confirms receipt of this document.
