Licensing Program Analyst (LPA) Maddox conducted an announced visit today and met with Denise Camps, Director for the purpose of conducting a Pre-Licensing inspection for an Infant/Toddler component for children ages 0 to 24 mos. The hours of operation will be: Mon through Friday from 6:30 am to 6:00 pm. Applicant is requesting to be licensed for 30 Infants (Applicant has decided to forego the Toddler option since a physical separation will be required). Together we measured 2 rooms for Infants. Measurements were as follows:

Room #1 (larger room):

Small side area: 14 X 17 = 98

Middle area: 27 X 16 = 418

418 + 98 = 516/35 = 14

Room #2 (crib room)

The measurements include the middle portion of the room excluding cribs

16 X 22 = 330/35 = 10

Outside Play area:

68 X 21= 1428/75 = 19

INDOOR ACTIVITY SPACE:

· The child care center was toured and found to be clean, safe, and sanitary to ensure the safety and well-being of children, employees and visitors


· Floors of all rooms have a surface that is safe and clean (carpet).
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Tables and chairs were present to meet the needs of the children.
· Drinking water is readily available via water pitcher for Sippy cups.
· There is/are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children:
· The isolation is located in the office for any child who becomes ill during the day.
· LPA observed carbon monoxide detectors (tested and operable) and smoke detectors(hard wired).
· The center has a working telephone
· Sign-in and out procedure (manually).
· The licensee shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.
  • Fully charged Fire extinguishers were observed throughout the center

The Infant play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. The infant yard surface is comprised of artificial turf grass. The entire yard is fenced, there are no bodies of water on the premises. LPA observed a locked storage shed to store play equipment. There is a swing set next to the Infant yard that will be off limits to Infants because it's not age appropriate. There is also another play area adjacent which is designated for School age children, the 2 yards do not have a physical separation. Staff must ensure they provide adequate Supervision while infants are outside to ensure Infants do not access school age play equipment. LPA measured the fencing surrounding the play yard, the measurements were exactly at 4 ft.

Drinking Water - staff will bring sippy cups out for Infants. Each cup shall be labeled with each Infants name.

RESTROOMS

There is no designated bathroom for Infants, LPA observed a diaper changing table with a portable sink within arms reach. Staff bathroom- there is a staff bathroom located hallway.

SUPERVISION: Applicant shall ensure no child is left without the supervision of a teacher at any time, Supervision shall include visual observation.

TRANSPORTATION: Will not be provided for Infants

CHILDREN’S RECORDS:


LPA explained all children's records shall be available to the Department to inspect, audit, and copy upon demand during normal business hours. Applicant states she is aware of the updated Safe Sleep requirements including form LIC 9227 and 15 checks (including documentation).

· LPA observed 10 cribs lined up on the perimeter of Infant Room #2. There are also mats available for Infants over 12 mos.


· Each mat shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.
· Mats should be wiped with a detergent/disinfectant weekly or when soiled or wet.
· Bedding/Linen will be stored separately and individually for each Infant in care.
  • Each crib, mat or cot shall be occupied by only one infant at a time.
  • LPA did not observe any bumper pads or any objects

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children.

A refrigerator shall be used to store any medication that requires refrigeration.

Applicant has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. FOOD SERVICES

Parents will provide the formula and center will provide snacks, the snack menu must be posted for parents review

· Pesticides and other similar toxic substances were not stored in food storerooms, kitchen areas, food-preparation areas, or areas where kitchen equipment or utensils are stored. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies.


· Food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.
· Trashcans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof.
· There is a refrigerator available to store any medication that requires refrigeration.
  • High chairs are present to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding.

POSTINGS:

Applicant was informed of the required forms that shall be posted after licensure including the telephone number of the local health department and information on child passenger restraint systems pursuant to Health and Safety Code section 1596.95(g) and Vehicle Code sections 27360 and 27360.5

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available.

Applicant is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B.

Fire Clearance has been received for the requested capacity of 30, however, with measurements taken today, center can accommodate a total of 25 children (15 in the large room and 10 in the crib room).

Needed Prior to Licensure: LPA observed hanging small metal brackets on the wall and some small holes in the wall that occurred as a result of removing shelves, LPA is requesting to remove the metal brackets and cover the holes to provide a safe environment for Infants.

Disinfectants, cleaning solutions, and other items that could pose a danger if readily available to children were stored in a cabinet in the School age girls bathroom with no lock in place. LPA is requesting Director place a child latch on this cabinet for the safety of children.
