Licensing Program Analyst (LPA) Maddox met with Site Supervisor Laura Flores for the purpose of conduct an unannounced pre-licensing inspection. There are 25 children present during this inspection. Center is requesting to add Infants to their licensed Preschool (X 153808909). The child care center will operate Monday – Friday 5:00a.m. – 5:00p.m. LPA informed Laura with the addition of the Infant component center will be converted to a single license.

INDOOR ACTIVITY SPACE:

· The child care center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors


· Floors of all rooms have a surface that is safe and clean (laminate vinyl and area rugs.
· A comfortable temperature for children shall always be maintained.
· Furniture and equipment are maintained in good condition, free of sharp, loose or pointed parts. There are a variety of age-appropriate equipment, toys, and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· Tables and chairs were present to meet the needs of the children.
· Drinking water: LPA observed water coolers with disposable cups.
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (Locked cabinet)
· There's are fully stocked first-aid kit(s) in locations accessible to staff but inaccessible to children that include thermometers.
· The isolation area is located in the office for any child who becomes ill during the day.
· LPA observed operable carbon monoxide detectors, smoke detectors (hard wired), and fire extinguishers are located throughout the facility and fully charged. · The center has a working telephone

· Staff shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted.

Safe Sleep - CCC and FCCH LPA discussed the safe sleep regulations with Site Supervisor Laura Flores and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. In addition, LPA printed out PIN 20-24-CCP which explains the most recently approved safe sleep regulations.

There were no cribs present, Site Supervisor, Laura Flores state they will care for Infants 12 mos to 24 mos. only which means they will only accept Infants (12 months and over) who are able to climb out of cribs and capable of rolling over on their own.

Infant: Outdoor activity space shall be physically separate from space used by children in the child care center and school-age child care center components.

LPA observed shaded rest areas on each play yard (there are separate play yards for Infants and Preschool children). Children have safe access to the play yard which is enclosed by a fence to protect children and to keep them in the outdoor activity area. LPA did not observe any bodies of water on the premises. The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. As a condition of licensure, the areas around and under high climbing equipment, swings, slides and other similar equipment shall be cushioned with material that absorbs falls (LPA observed rubber matting under outdoor play equipment).

Cots used for napping shall be maintained in a safe condition. Cots must be wiped with a detergent/disinfectant weekly or when soiled or wet and shall be be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child.

Each licensee shall have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available. Contingency plans for action during fires, floods and earthquakes including, but not limited to, the following: FORM: LIC 9148

Infant Classroom measurements (Room A):

The Indoor total measurements: 501 sq. feet, which will accommodate Director's request for 12 infants.

Preschool Classroom measurements (Room B & C):

The Indoor total measurements: 1094 which will accommodate the capacity of 30 Preschool children.

Infant Outdoor Play measurements are:

Infant Changing Tables

LPA observed changing table(s) to have a padded surface no less than one-inch thick and covered with washable vinyl or plastic. The changing table had raised sides at least three inches high; maintained in good repair; and placed within arm's reach of a sink.

Restrooms: Infant bathroom has 2 kids size toilets and sinks.

All toilets, hand washing and bathing facilities shall be maintained in safe and sanitary operating condition. Additional equipment, aids and/or conveniences shall be provided as needed in centers that serve children with physical disabilities.

HEALTH RELATED SERVICES:

Medications shall be kept in a safe place inaccessible to children. Site Supervisor states staff will dispense prescription medications only.

Director has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

Food Service: Center will provide breaksfast, lunch, and snacks

Infant: Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. High chairs or other appropriate seating equipment shall be used to seat infants during feeding. Infants who are unable to sit unassisted in a high chair or other seating equipment shall be held by a staff person or other adult for feeding. Bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date. The infant care center shall not serve honey or corn syrup to any infant.

All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. All food shall be selected, stored, prepared and served in a safe and healthful manner.

All foods or beverages be stored in covered containers at 45 degrees F (7.2 degrees C) or less.

All kitchen, food-preparation and storage areas shall be kept clean and free of litter and rubbish; and measures shall be taken to keep all such areas free of rodents and other vermin.

Full-day programs shall ensure that each child has a lunch. Half-day programs shall offer a midmorning or midafternoon snack.

The licensee shall post the PUB 393 (8/02), Child Care Center Notification of Parents' Rights Poster in a prominent, publicly accessible area in the child care center at all times.

Menus shall be in writing and shall be posted at least one week in advance in an area accessible for review by the child's authorized representative. LEAD TESTING

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP (9/27/2022).

The following items were discussed:

Director is aware of form the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed: LPA requested a 2nd request for the fire department, initial request was sent 6/22/26.

An exit interview was conducted with the above items discussed and a copy of this report was provided to the Site Supervisor Laura Flores. Final license determination will be made upon review by the Licensing Program Manager and fire clearance.
