On 01/22/2025 Licensing Program Analyst at 1:33 PM (LPA) Olena Maico made an unannounced visit to the above facility for the purpose of following up on the information received from CCIB on the emergency referral 0115-5212-3481-2164028. It was alleged that C1 was in placement at the facility somewhere between June-July 2024 and when on AWOL she would engage in sexual activity with the 20 year old man . No information was provided for the man.
Prior to visiting the facility LPA reviewed SIRs involving C1 for the months of June and July since no specific dates were given for the incidents. LPA found the SIRs to be properly submitted and completed with the detailed information of the AWOL dates and the LAw Enforcement notifications, as well as when the MET teams were called and child was taken for the 51/50 hold. LPA has not found any evidence of the child disclosing the information of inappropriate sexual activity with the man to the facility. Also, when reading SIRs for AWOL, the child was most of the time in the staff's view and maximum was absent for about 20 min and then the staff would find her in the fire station and call MET.
LPA spoke with the Administrator and also Administrator stated no information for the inappropriate sexual interaction with the 20 year old man was disclosed by C1.
LPA has not found the evidence of lack of neglect or supervision form the facility. No deficiency has been cited.
The copy of the report and the LIC 811 was given and discussed with the Administrator.
