25 | On 09/20/2024, Licensing Program Analyst (LPA) Octavia Nolan and Licensing Program Manager (LPM) Gloria Reyes conducted an unannounced case management inspection. LPA and LPM met with Site Supervisor Shoua Moua. LPA and LPM toured the facility inside and outside and took a census.
On 09/13/2024, the facility notified Community Care Licensing Office (CCL) via telephone regarding an Unusual Incident. On 09/12/2024, Child #1 went to wash their hands in the restroom with Teacher #1. Teacher #1 was putting an empty plastic bottle on top of the cabinet, when it fell and cut Child #1’s nose. Teacher #1 immediately attended to Child #1 and administered first aid. Child #1’s parent was called, and they were picked up approximately 30 minutes later. The parent took Child #1 to the doctor, where they were treated without complications and released. The parent kept Child #1 home the next day. Child #1 returned to care on Monday, September 16th and is doing fine.
LPA and LPM inspected the area where the incident took place. There are no hazards present in the restroom and no items were on top of the cabinets. This appears to be an isolated incident and staff took appropriate measures to address the child’s injury following appropriate Agency policies and procedures.
Exit interview conducted and report was reviewed with Site Supervisor Shoua Moua. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. |
