On 03/04/25, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Site Supervisor, Maria Lopez. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 02/12/25 regarding a child’s personal rights being violated.

Incident occurred at the facility where a child alleged that staff #1 had grabbed Child #1 by the wrist. Through staff interview it was revealed that an incident did occur where staff #1 squeezed child #1 wrist because child #1 did not want to move out of the way during recess. Facility also failed to report the incident to child #1 parents. Site Supervisor spoke with Staff #1 regarding personal rights of a child and reporting requirements. Staff #1 has resigned from the facility.

LPA reviewed personal rights of children in care, reminding Site Supervisor that staff must comply with personal rights of a child and not violate their rights. LPA also reminded Site Supervisor to report any type of incidents to parents/authorized representatives.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, this deficiency is being cited on the attached LIC 809D.

Exit interview conducted with Site Supervisor, Maria Lopez. Appeal rights were given.

This report is to be made available to the public upon request.
