25 | On 04/26/24, Licensing Program Analyst Denisia Jimenez conducted an unannounced case management inspection to discuss an incident report that was submitted to Community Care Licensing regarding an incident that occurred at the facility on 04/12/24. LPA met with Owner Laurae Raths.
Staff #1 arrived at the facility with 7 children who were picked up from school. 4 children got out of the vehicle and closed the door. Staff #1 assumed that all 7 children had gotten off from the vehicle because the doors to the vehicle were closed. Staff #1 entered the gate and realized that only 4 children were lined up and went back for the other 3 children who were in the vehicle. LPA reviewed the footage and did see 4 children get out of the vehicle and Staff #1 walk through the gate. The camera goes out of view from the gate and approximately 30-40 seconds pass by until Staff #1 goes back out the gate to the vehicle and gets the other 3 children. Based on the camera footage it is undetermined if Staff #1 left the immediate vicinity from where the vehicle was parked at while she was assisting the other 4 children. Staff #1 contacted the 3 children’s parents and spoke with them individually and explained to the parents of what had occurred. Per Laurae training was provided to the staff so this incident will not happen again and will do additional training.
This appears to be an isolated incident and staff took appropriate measures to address the staff, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations no deficiency was cited during today’s inspection.
