Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 Year inspection on this date. LPA met with Christie Benitez, Director, and explained the purpose of the inspection and was guided on a tour of the facility. Facility is currently licensed for a capacity of 49 children. There were five teachers on the premises along with the director. Parents and authorized adults use an electronic sign in and out App (Brightwheel App), The devise is capable of printing sign in/out sheets upon request of licensing representative or other enforcement agencies. The facility consists of 3 classrooms; Triangle, Square, and Circle. The facility operates Monday through Friday, from 9 a.m. to 4:30 p.m. The school offers a morning (9:00am-12:30pm) and afternoon (1:30pm - 4:30pm) sessions in Circle room. The facility also uses one classroom that is not licensed as a "Parent and Me" room for children between the ages of 18 months to 2 years 9 months. These children are present with their parents and attend either 1 or 2 days a week for 2 hours each session. The room is separate from the preschool rooms and has its own outdoor space. Facility is operating within capacity limitations There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible.A walk through of the classrooms was conducted, the classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation. Isolation area for sick students is in the Director’s office. LPA observed adequate arrangements for isolation and care of ill children. Page1

LPA inspected and observed 3 clean bathrooms. The Circle room has one bathroom containing 2 toilets and 2 sinks. There are two bathrooms accessible from outside the classrooms, containing a total of four toilets, two urinals and 3 outside sinks. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate. Furniture and equipment were inspected for age appropriateness and good repair. All rooms are clean and safe. Telephone service was verified. Heating, lighting, and ventilation are adequate. All rooms have central air conditioning and heat. There are cubbies for children's belongings used to store personal items. LPA observed age appropriate toys and materials. Drinking water is available inside the classrooms and outside in the form of a water bottles. LPA observed a sufficient quantity of mats available for napping children in the Triangle room. The rest period is offered for the children who attend the full day program. The bedding is stored separately and properly in bags in a storage area for bedding. Outdoor play equipment, bicycles and toys were observed to be in good condition. The outdoor yard consists of play structures, swings and various climbing structures, all securely mounted over sand. There is a cement area for bicycle play. Shade is available through the use of outdoor canopies and trees. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. Fencing around the perimeter of the play area is at least four feet high. There are water tables on the yard that were observed to be covered and empty. There is a play structure with a water spout that is turned off when not in use. Disinfectants and cleaning solutions were observed to be inaccessible to children above the cabinets in the classrooms. The cleaning closet is located in the rear yard and is latched. Poisons are locked in an outdoor shed used by maintenance. Page 2

There is a clean kitchen with a microwave oven and sink with hot water. There is a larger refrigerator in the Triangle room which is inaccessible to children. The facility provides two snacks and all children bring their own lunch. Food is available in the event that a child's lunch is forgotten. Menus are posted. LPA observed an appropriate amount of food and snacks. The chemicals are kept separate from the food. Trash cans used for solid waste disposal contain a tight fitting lid.
The facility has several pets, including a guinea pig, a tortoise, and 5 chicken.

Children are inspected for illnesses as they arrive. A review of medication policy indicated that medication is administered and only with parent's written permission. Director administers the medication and documents the dosage, date and time onto a log. Medication is properly labeled and stored in its original container. There is a separate area for isolation and care of ill children in the director's office. There is a mat available for each ill child. An isolated toilet and sink are available for ill children.

The facility is equipped with a working smoke detectors and Carbon Monoxide detector. LPA observed three fire extinguishers (2A10BC) located throughout the facility with service date of 03/24/2024. First Aid Kit was observed to be complete. Fire Drill log was observed to be complete and current. .

The facility has sufficient mats for napping. Each mat is occupied by only one child at time. LPA reviewed a sample of children’s files and observed files were complete. Page 3

CPR, Pediatric First Aid (completed on 09/02/2022), and Mandated Reporter Training certificates ( completed on 10/01/2023) were observed to be current. The name of the child care center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department. LPA reviewed a sample of staff files and observed files were complete.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
