25 | On 07/26/24, Licensing Program Analyst, (LPA) Shushanik Safaryan conducted an unannounced Case Management inspection for the purpose for the following up on an unusual incident report. Upon arrival at 08:30 am , LPA met with Director, Ellen Veselack , to whom the purpose of the inspection was explained. Tour of the facility was provided. There were 31 children and 11 staff members were present during the inspection.
On 07/24/24 an unusual incident report was submitted to the Department regarding a child sustaining an injury on the facility grounds. Child was playing in the classrooms dramatic play area and fell from the table . Per staff members child was attended immediately by staff #1 and ouch report was created for the child on the day of the injury .Child was taken to the urgent care by a parent where cast was placed on her elbow . During the visit LPA inspected the area where incident took place . Table was removed by the staff after the incident . Per staff members table was there temporarily from the different class and was returned to that class . Based on the interviews 14 children and 2 staff members were present during the incident. Per staff #1 child climbed on the table and was reminded to keep her feet on the floor when incident happened .Per staff # 2 she observed child #1 climbing on the table and observed staff #1 reminded the child to keep her feet on the ground .Per child #1 she observed staff #1 sitting at the table when she was playing and fell when she was coming down from the table .Based on the information obtained and observation , LPA determined that there was adequate supervision on the day of injury and no citations issued at this time .
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
Exit interview conducted with the Director and Copy of Report provided.
