13 | On 09/19/2023 at 9:40 a.m. LPA Whitmore initiated the complaint investigation and met with the Licensee Edith Zsigmond. LPA toured the facility indoors and outdoors. LPA observed 5 children in care and 2 Adults. LPA obtained the following documents, Roster, and Mission Statement.
On 12/6/2023 2023 at 9:40 a.m. LPA Whitmore conducted a visit to complete the investigation and deliver findings. LPA Whitmore met with the Licensee Edith. LPA toured the facility indoors and outdoors, observing proper teacher/child ratios with 6 total children in care and 2 Adults.
The Department conducted a full investigation, which included staff interviews, interviews with relevant parties, as well as a record review which included documentation related to the allegation. LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegation of Personal Rights- Licensee inappropriately disciplined a day care child. Although the allegations may have happened or are valid there is not a preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegation is deemed unsubstantiated. An exit interview was conducted, a copy of this report, appeal rights along with Notice of Site Visit were provided. |
