25 | Licensing Program Analyst (LPA), Mayra Cota conducted an unannounced annual visit today. LPA met Armine Kim, Director, and explained the reason for the visit.
The facility is licensed as an Adult Day Program with a capacity to serve 131 clients of which 20 may be non-ambulatory. The facility is in a commercial area of Pasadena and consists of the following: a lobby, (4) offices, kitchen, dining area, (3) restrooms, and participant storage locker area. The facility also has (7) classrooms which include the computer room, music room, wellness room, activities room and conference room.
During today’s visit, LPA observed the following:
Facility is in good repair inside and out. All rooms in the facility have sufficient seating for staff and participants and furniture is in good repair. Walkways, passages, ramps and exits are free of debris and obstructions. Activity schedule is posted, and the facility has activity materials and equipment which is accessible to participants. Kitchen and dining area are clean, and sharps and cleaning supplies are kept locked in a kitchen cabinet and inaccessible to participants. Kitchen appliances were observed clean and operable. Refrigerators are available for participants to store their packed lunches. Participants are required to bring their lunch from home; however, the facility conducts planned cooking activities once a week and provides snacks during special events. Restrooms were observed clean, and water temperature tested during visit, measured between 109.5-112.7 degrees F. No pools or large bodies of water were observed. The facility’s outdoor environment has tables and benches, and a shaded area was also observed. Participant recyclables are kept in the patio area; however, the bags with the recyclable items are kept near the patio area for the participants. Fire extinguishers were observed throughout the facility and are charged and operational. The smoke and carbon monoxide detectors were tested and were also observed operational.
