On 07/09/24 , at 8:30 am , Licensing Program Analyst (LPA ) Shushanik Safaryan conducted an unannounced visit regarding to deliver findings for the above allegation received by the Department.
During the visit , LPA met with Facility Representative , Patricia Chambers , to whom the purpose of the visit was explained. Facility Representative guided LPA on the tour of the facility . This is the School Age program that operates from 07:00 am to 06:00 PM and consists of 3 classrooms . During the inspection LPA observed 34 children with 4 staff members.
Allegation states: Staff do not ensure vehicles transporting day care children are properly maintained.
During the 10-day visit on 06/18/2024 ,LPA inspected the vans . During the inspections, two of six vans had a dashboard warning .(Pictures were taken ).Per Director , the van with dash warning regarding air bag services was in limited use , the other van with dash warning regarding tire pressure , staff was instructed to take visual and dash confirmation of air pressure until it could be repaired. During the visit LPA interviewed seven staff members .

California Code of Regulations, (Title 22, Division & Chapter number), are being cited on the attached LIC 9099D.”)

Exit interview conducted with Facility Representative , Patricia Chambers. The Notice of site visit , report and Appeal Rights were explained and provided to Facility Representative on 07/09/2024.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.

On 06/18/2024 LPA observed in two out of six vans dash warning. Based on the interviewes cars were driven with the sensor warning .
