The child #1 was later found by another staff member inside the van.
The Director admitted that she did not notify the Department about the incident and did not file an incident report. While the Director claimed that the child’s parents were notified, she could not provide a copy of the incident report to the LPA. The LPA also did not find an incident report in the child’s folder.

The LPA interviewed child #1 and they could not recall specific details about the incident but mentioned that they had fallen asleep. Parent #1 recalled the incident from last summer and confirmed that the facility had informed her about the child being left unattended in the vehicle. According to Parent #1, the child was fine after the incident and did not require medical treatment.

Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED.
