Licensing Program Analyst (LPA) Tatiana Bickham conducted an unannounced site visit on 04/15/2026 at 11:00 AM to this facility to deliver findings on the above-mentioned allegations. Upon arrival, LPA met with Director Jennifer Kim, and explained the purpose of the visit. There were 7 preschoolers being supervised by 1 staff at the time of visit.

During the course of the investigation, interviews were conducted with Director, parents, children, and staff. LPA reviewed the parent handbook and copies of the children's and staff rosters were also obtained and reviewed.

Per Reporting Party, due to lack of supervision, child bit another child, staff did not report incidents to authorized representative, and saff left child in soiled diapers/clothing.

Allegation: Lack of supervision; child bit another child

Per interview with the Director, no biting incidents have occurred. The Director stated they do not have a written biting policy. If a biting incident occurs, the facility applies an ice pack and notifies both parents. The Director also stated they do not have a written supervision policy, but reported staff are always with the children.

Per interview with staff, the center has a biting policy that includes explaining to children not to bite, separating children for 1–2 minutes, cleaning the affected area, applying ointment, and providing parents with a written note.

Per interviews with parents, responses were inconsistent with the above-mentioned allegation.

Per interviews with children, the children stated they enjoy coming to school and have not experienced any biting incidents.


Allegation: Staff did not report incidents to authorized representative

Per interview with the Director, if an incident occurs, parents are either called or provided an ouch report.

Per interview with staff, parents are notified of incidents at pick-up or via phone call.

Per interviews with parents, responses were inconsistent regarding the above-mentioned allegation.

Children were not interviewed for this allegation as it is not pertinent to children in care.

LPA asked for copies of recent ouch reports/written reports given to parents. Per the Director, the facility does not keep copies, the reports are given directly to parents.

Allegation: Staff left child in soiled diapers/clothing

Per interview with the Director, diaper change occurs every 2 hours with additional checks every hour. Parents provide diapers and wipes. The policy is included in the parent handbook. Staff receive hands-on training from the Director regarding diaper changing. Per the Director, no child has been left in a soiled diaper.

Per interview with staff, children are changed every 2 hours or as often as necessary. Children are also changed before leaving the facility.

Per interviews with parents, responses were inconsistent with the above-mentioned allegation.

Children were not interviewed for this allegation as it is not pertinent to children in care.


LPA requested a copy of the parent handbook from the Director and received an enrollment packet. LPA was informed this packet serves as the parent handbook. LPA did not observe any section dedicated to diapering in the enrollment packet.

Based on the investigation conducted, there is insufficient evidence to support the above-mentioned allegations to be true. Therefore, although the allegations may have happened or are valid, there is not a
preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.

The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

Exit interview was conducted with Jennifer Kim, Director and appeal rights were provided.
