Licensing Program Analysts (LPA)s Monica Ruiz and Stephanie Li conducted an unnouncesed visit to the above facility for the purpose of conducting a complaint investigation for the above allegations. Upon arrival LPAs met with Director Kristy Knight, who arrived at the facility at 10:15am and led LPAs Ruiz and Li on a tour of the facility. LPAs observed the following: Teddy Bear Room had 9 children and 2 staff, in the Wildflowers Two-year Old Classroom there were 2 staff with 1 children.

During todays investigation, LPAs toured classrooms and observed completed construction, reviewed and obtained copies of staff timesheets for July, reviewed staff files and qualifiactions, obtained photos of Dojo messages to parents for classrooms in 'grandmas house', and interviewed 3 staff.

During record review, LPA's found that 2 assistant teachers were left alone with 11, 2 year olds. Current transcripts were reviewed and assistant teachers are not qualified to be left alone without a fully qualified teacher, which poses an immediate risk to children in care. * Continued on next page...

A deficiency is being issued on the following page LIC9099D. All deficiencies need to be cleared to protect that health and safety of children in care.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted, report and appeal rights was reviewed with Director Kristy Knight.

Per director, assistant teacher has 9 current units and is waitlisted for the missing child development course. Director stated she teaches CHDV at Chaffee and will ensure to enroll assistant teacher into needed CHDV course. Director will email confirmed enrollment to LPA M Ruiz by 10/13/2025.

Based on observation and record review, 2 assistant teachers were left alone supervising 11 two-year olds without a supervising qualified teacher, which poses an immdeiate health and safety risk to children in care.
